- Request for Information. If you have any questions about your project, and want to be sure it is eligible for presentation, you can request advice and a ruling from the ethics committee of your Regional Science Fair:
- Download this Request for Advice or a Ruling.
- Fill it in.
- Follow the instructions at the end of the form.
- Research Proposal. Before starting the research, students must submit a Research Plan to the Chair of the Ethics Committee of their Regional Science Fair.
- Informed Consent. Before humans participate in a project, they must give their informed consent.

- Informed Consent - Letter of Information - blank Use it to explain your project. Give one to every participant to keep.
- Informed Consent – Permission Form Every participant must sign one. The Adult Supervisor must store them securely. These must be shredded after the science fair season is over.
- Approval of Significant Risk Projects. After you have finished your project, but before you arrive at the science fair:

- Fill it in and get it signed.
- Follow the instructions at the end of the form.
