Human Participation - Significant Risk Forms

  1. Request for Information. If you have any questions about your project, and want to be sure it is eligible for presentation, you can request advice and a ruling from the ethics committee of your Regional Science Fair:
    1. Download this Request for Advice or a Ruling.
    2. Fill it in.
    3. Follow the instructions at the end of the form.
  2. Research Proposal. Before starting the research, students must submit a Research Plan to the Chair of the Ethics Committee of their Regional Science Fair.

  3. Informed Consent. Before humans participate in a project, they must give their informed consent.

    2. Informed Consent - Letter of Information - blank Use it to explain your project. Give one to every participant to keep.
    3. Informed Consent – Permission Form Every participant must sign one. The Adult Supervisor must store them securely. These must be shredded after the science fair season is over.
  4. Approval of Significant Risk Projects. After you have finished your project, but before you arrive at the science fair:

    2. Fill it in and get it signed.
    3. Follow the instructions at the end of the form.
